Sycamore Springs is a 48-bed behavioral hospital located in Lafayette, IN. The hospital offers inpatient and outpatient mental health and addiction treatment to adults, adolescents and their families. As an Assessment Supervisor, you will be a member of the clinical services team at Sycamore Springs. Our team drives a “Clinical First” attitude to help fulfill our mission of “Changing People’s Lives®”. The responsibility of the Supervisor is to perform the evaluation and admission process in conjunction with the on-call physician. Assists the Director of Assessment in directing all department activities and monitoring the quality of services provided. 
 
 
Position Details:
  • Facilitate response to request for services, including the initial response, the assessment and referral process, the designation of appropriate level of care, admission into services and appropriate follow up activity.
  • Conduct initial evaluation, initiate the assessment, document patient symptomology, functioning, and needs
  • Provides information to MD for LOC determination.
  • Clinical supervision for LMHC and LCSW available/possible. 
  • Educates and coaches staff and acts as a point person for clinical decisions. Reports all issues and incidents to the Director of appropriately.
  • Ensures staff has completed all needed documentation, call logging, insurance calls, and additional tasks prior to the end of shift.
     
Requirements:
  • License pending Master’s Degree in Social Work or Counseling required, RN licensure may be used in place of master’s degree.
  • Valid Driver’s License and a favorable Motor Vehicle Report and proof insurance when using own auto.
  • CPR and CPI Certification within 30 day of employment
Experience:
  • Prior experience in a psychiatric healthcare facility working with adult and geriatric patients preferred.  Must have strong clinical assessment skills.
