Requirements (Education, Experience, Licensure, Certification): 
  •  Master's Degree in Social Work (or Human Services related) or Bachelor's degree in Social Work (or Human Services related) plus six (6) months paid relavant experience or Bachelor's degree in Social Work and successful completion of a field placement at a public children's services agency ({CSA).
  • Must possess a valid drivers license issued in the state of residency. 
  • Must have the use of an insured vehicle. 
Job Duties (Summary):
  •  Responds to reports of physical abuse, neglect, self neglect, emotional abuse and exploitation to adults aged 60 and older living in an independent living arrangement.  Responds to the priority (emergency vs. non-emergency) of the report within the mandated time frame. Conducts face to face interviews with the alleged adult victim. Responsible for interviewing other known persons or agencies that may have knowledge of the abuse, neglect or exploitation.  Completes all investigative activities as designated by law. Responsible for determining the validity of the report. Responsible for assessing the risk to the adult who is subject of the report. Consults with the Prosecutors Office and seeks Court intervention as needed. Testifies in Probate Court Proceedings. Educates consumers of services available in the community. Completes referrals to community social service agencies when appropriate. 
  • Responsible for determining the need for on-going Adult Protective Services.  Develops written Protective Service Plans. Conducts on-going face to face visits with the alleged adult victim to reduce identified risk factors within the household.  Responsible for re-assessing the level of risk and amending the Protective Service Plan as needed.  Responsible for the delivery of On-going services or arrange on-going service delivery via referrals to community agencies.
  • Participates in supervisory conferences, unit meetings, staff development including CORE Training as well as committee participation.  Conducts Community presentations.  Provides education to the community and other agencies on a client’s right to self-determination and to the consumer’s right to a least restrictive living environment. 
  • Responsible for maintaining written documentation of investigative activities and on-going Adult Protective Services.  Paperwork includes but not limited to Client Contact Notes, Investigative Summary, Family Assessment, Assessment Summary and Protective Service Plans.  Responsible for other mandated paperwork as outlined by federal and state regulations and the Council on Accreditation. Responsible for maintaining Adult Protective File and obtaining service provider reports when appropriate. 
  • Performs other related duties as assigned.
