The Home Health LPN Clinical Liaison coordinates case management from facilities/hospitals to home health ensuring accurate information and efficient admissions of patients.

  • Communicates, collects, and provides information from hospitals and other organizations regarding new and existing patients with Shepherd of the Valley Home Health services. 
  • Monitors and follows Shepherd of the Valley Home Health patients in the hospital. Relays progress and updates to Shepherd of the Valley Home Health. 
  • Meets with new patients and family members to collect information prior to admission and assess the living environment and family support situation.
  • Communicates with physician offices in regards to patient care and relays this information to Shepherd of the Valley Home Health.
  • Visits physician offices to maintain relationships and obtain signed orders and other needed information on patients. 
  • Maintains and facilitates working relationships with hospitals, physician offices and others.
  • Answers requests for information on Shepherd of the Valley Home Health services from prospective customers and others.
  • Maintains the confidentiality of all resident, staff and organization information.

  • Must hold a Nursing Degree from an accredited program or college/university
  • Must hold a current, unencumbered, active Ohio LPN license 
  • Must have CPR certification.
  • Must have at least 1 year experience in Long Term Care
  • Experience with home care, discharge planning or insurance billing is a plus.
