Program Coordinator/Therapist
NORMAL WORK DAYS/HOURS: Monday thru Friday 8:00 am to 4:00 pm; 24/7 On-call responsibilities
Location:  Stonebrook Apartments
 
This mental health professional is responsible for the operation of the Stonebrook MOM Apartment Complex (30 beds) and for providing clinical services for seriously mentally ill clients residing in the Apartment Program. Supervise the Home Coordinator, direct care staff and co-supervise the nursing staff.
 

PRIMARY JOB FUNCTIONS:

Provides clinical services such as Basic Living Skills training, Case Management, Family Support, etc., to the residents of the Apartment Program.

Completes documentation of these services in a timely manner.

Submits documentation and billing forms to the Assistant Director of the MI Residential Program on a weekly basis.

Maintains an emergency call list.

Posts monthly activity schedule.

Coordinates monthly Psychiatrist and on-site Treatment Teams.

Coordinates and/or provides weekly Group, Individual and/or Family Therapy Sessions as needed.

Arranges provisions for coverage during off time.

Appropriately responds to phone calls and written correspondence related to apartment complex.

Responsible for reporting maintenance and upkeep needs of buildings and any equipment assigned to Apartment program.

Checks apartments and furnishings at least once a month.

Monitors apartments to ensure that residents keep dwellings clean and remain in compliance with State Department of Mental Health/Mental Retardation standards. Reports any consistent non-compliance to Director of Mental Illness Residential Program.

Effectively responds to maintenance requests within his/her capability in a timely manner.

Monitors for safe and secure conditions on grounds and buildings.

Ensure the vehicle assigned to program or stationed at the home is maintained in accordance with the Agency’s Vehicle Maintenance Policy.

Responds in an effective and cost-efficient manner in the event of any emergency.

Reports all emergencies to Director of Mental Illness Residential Program or other management personnel as indicated.

Provides support services to residents:

Provides Basic Living Skills Training/Case Management/Family Support Service

Effectively deals with chronic and acute psychiatric emergency’s

Coordinates with appropriate mental health providers when necessary for emergency care

Provides appropriate support for routine adjustment difficulties (anxiety attacks, etc.)

Maintains an adequate knowledge of psychiatric medications to provide accurate information to clients and to recognize negative side effects.

Coordinates with appropriate agency services (food stamps, social security, etc.) to ensure resident's needs are met.

Effectively discerns client regression and makes appropriate referral by notifying appropriate personnel.

Performs administrative duties as required

Maintains appropriate client information sheet with releases for service providers

Completes paperwork in a timely and accurate fashion

Exchanges information with supervisor or co-workers as needed

Provides emergency transportation to residents

Maintains documentation of monthly inspections

Serves as a liaison with community mental health centers, other JBS programs, Department of Mental Health and other social service agencies:

Attends DMH and Agency training programs and other meetings as required.

Ensures that clients comply with programmatic requirements.

Ensures good relations with community and neighborhood.

 

MINIMUM QUALIFICATIONS:

Masters degree in Human Services required. Two years experience working with the mentally ill required.

Valid Alabama driver's license and driving record acceptable to agency automobile insurance carrier for inclusion on agency policy.

Direct Care experience working with a wide variety of people with multiple mental health diagnoses, including those with a SMI and legal issues.

Experience working with Consumers with an NGRI status. (i.e. working knowledge of the NGRI monitoring and written reporting system to Circuit Courts as ordered in a Conditional Order of Release, able to testifying in Circuit and Probate Courts if necessary).

Certified thru the Nursing Delegation training program to provide medication assistance.

Minimum of 2 years Supervisor Experience, five years perferred.

Able to be on call 24/7 to coordinate any problems
