Reporting to the Practice Manager, the Registered Nurse is responsible and accountable for delivering excellent patient focused, evidence-based nursing care in a specified field of practice (Chronic Disease Management). The Registered Nurse is a member of the multidisciplinary health care team and uses contemporary standards and evidence to underpin practice.

Role and Responsibilities
  • Consult with patients via face to face, phone or tele-health to guide them through the clinic process
  • Provide excellent standards of evidence-based nursing care and documentation to facilitate quality patient outcomes, in a specialist area of nursing (Chronic Disease Management) in accordance with relevant organizational standards, policies and procedures. Risk Profiling and care- planning required
  • Assist prescribing HCPs with on-site management services, administrative and patient support
  • Perform medical outreach and presentations within our network of HCPs
  • Provide relevant contemporary information and education to patients and families/care-givers to promote independence and control over their health
  • Participate on ongoing development programs, including research and educational programs
  • Adhere to defined professional practice standards and code of ethics, health and safety policies and procedures to ensure high quality and safe services
  • Practice in accordance with relevant legislation, Acts and agreements, workplace health and safety, equal employment opportunity and anti-discrimination practices
  • Comply with any reasonable policy or procedure to ensure compliance with health and safety requirements
  • Participate in activities related to the recruitment and retention of Clinic patients. This may include delivering education sessions
  • Participate in improving patient care and adherence to relevant standards 
  • Participate in quality initiatives aligning evidence based nursing practice with all components of the Clinic quality assurance/monitoring processes
  • Preceptor, mentor and share information with peers to develop their potential
  • Utilize a range of Clinic specific information and technology   systems like Clinic to Cloud, Microsoft and others to support effective decision making and optimize patient outcomes

Qualifications 

  • Registered with AHPRA with at least 2 years of post-registration experience as a registered Nurse or Enrolled Nurse
  • Experience with Chronic Disease management and associated care plans
  • Highly detailed and organized
  • Demonstrated understanding of evidence informed practice and critical thinking
  • Demonstrated commitment to person centered care and NSW Health Core Values
  • Demonstrated effective interpersonal skills, excellent written and verbal communication skills within a multidisciplinary team
  • Evidence of participation in own professional development and commitment to maintaining competence and enhancement of skills, and willingness to apply and share professional expertise with colleagues
  • Evidence of computer literacy and understanding of relevant health related information systems
  • Experience and/or interest in pursuing further training in clinical research
  • A strong team focus and personal work ethic
  • Record keeping and confidentiality that meets professional standards
