Position Title:  Family Nurse Practitioner
 
Primary Service Center: Springfield, Ohio (South Limestone Location)
 
FLSA Status: Exempt (Salary)
 
Work Hours: Full-time, 40 hours per week, some evening may be required
 
Position Summary and Responsibilities:  Full-time position responsible for medical examinations, counsels patients and develops health care plans.
 
  • Collects and evaluates medical histories of patients and maintains/updates patient records.
  • Performs physical examinations, interprets diagnostic and screening test results.
  • Prepares reports and complies statistics from laboratory logs.
  • Completes patient referral forms.
  • Performs patient follow-ups to insure proper health care.
  • Identifies and discusses medical practices with patients/family and counsels individuals on health care and chronic condition management.
  • Develops and initiates care plans.
  • Provides/coordinates nutrition consultation.
  • Monitors health program progress.
  • Prepares and maintains records and reports.
  • Attends team/staff meetings and serves on temporary committees as requested.
  • Maintains and increases knowledge and skills through attendance at meeting, conference, training seminars and in-service training.
  • Explore new models of health care through continuing education, networking with other centers and working creatively with a multidisciplinary treatment team.
  • Know and adhere to all policies and procedures that pertain to the job position

  • Master’s Degree in Nursing.
  • Registered Nurse Practitioner in the state of Ohio.
  • Two years related experience working with families and children.
  • Experience in Mental health preferred.
