Pathways is a halfway house that is designed to assist women involved in the criminal justice system reintegrate into the community.  This role will be primarily providing services to women in a halfway house, many of them with trauma needs.  This position will also have an opportunity to work with males in a halfway house setting.  The Clinical Service Provider will provide excellent customer service and maintain a caseload of clients to assist them to more effectively function in the community or to prepare them for reintegration into the community.  Duties and additional experience or requirements may vary by position.  
  • Under supervision; conducts, interviews and administers assessments and screening instruments and collects psycho-social information.
  • Maintain caseload and productivity through regular contact with clients.
  • Monitor and assess progress per agency policy, regulatory requirements and best practices.
  • Meet with clients individually or in a group to assess needs, collaborate with clients, families and other case managers and services involving site staff.
  • May provide but is not limited to: crisis intervention, advocacy, medication monitoring, transportation assistance, financial assistance, referral and linkage to community resources. May transport clients.
  • May provide Case Management duties.
  • Prepare clinical reports and individualized service plans, utilizing agency electronic clinical documentation software.
  • Assure documentation is in compliance with contract, regulatory and agency requirements and best clinical practices.
  • Efficiently operates personal computer to enter, edit and/ or retrieve data and to produce reports and documentation.
  • Effectively works as a team member, including participating in all required trainings, ensure the safety of others and participate in Peer Review/CQI process.
  • Bachelor’s degree in Behavioral Science or related area or;
  • Equivalent combination of experience consist and training
  • Complete an internal training and pass a competency test on electronic clinical documentation software during first eight weeks of employment
  • A valid driver’s license and current auto insurance may be required
  • Obtain Chemical Dependency Counseling Assistant (CDCA) within sixty days of hire
  • Previous work experience in court and corrections
  • Will be required to work 1 late night per week and 1 Saturday per month
