Walnut Hills Adult outpatient clinic provides community psychiatric supportive treatment to adults in the community.  This program is designed to assist adult clients whom in their behavioral health and in accessing needed community based services. We offer mental health services, including individual, group and family counseling.
The Adult Behavioral Health Case Manager will provide mental health and substance abuse services to assist clients to effectively function in the community.
  • Maintain caseloads and productivity by ensuring regular contact with clients in community and office.  
  • Monitor and assess progress per agency policy, regulatory requirements and best practices.
  • Meet with clients individually to assess needs; collaborate with clients, families and other case managers and clinicians.
  • May provide, but is not limited to: crisis intervention, advocacy, medication monitoring, transportation assistance, financial assistance, ADL assistance, referral and linkage to community resources. May visit, monitor and transport clients.
  • Prepare clinical reports and individualized service plans, utilizing agency electronic clinical documentation software.
  • Assure documentation is in compliance with contract, regulatory and agency requirements and best clinical practices. 
  • Efficiently operates personal computer to enter, edit and/ or retrieve data and to produce reports and documentation.
  • Effectively work as a team member including participating in all required training, ensure the safety of others and participate in Peer Review/CQI process.
  • Bachelor’s degree in Behavioral Science or related area or;
  • Equivalent combination of experience and training
  • A valid driver’s license and current auto insurance may be required
  • Experience with behavioral health population or case management
  • Communication skills

  • Computer literacy skills
  • Case Manager willl provide case management services to male and female adult clients. 
  • Clients may have co-ocurring diagnoses and various needs, including treatment, housing, and community support. 
  • Applicant must be able to communicate with clients in an effective and efficient manner. 
  • Applicant must be able to balance a caseload and documentation requirements. 
  • Applicant must be able to climb 2 flights of stairs.
