Health Care Access Now: Community Care Coordinator

The Health Care Access Now Community Care Coordinator will provide basic family and individual assistance to resolve social and medical barriers to health care and related services;  work with individuals to complete prioritized pathways. The Community Care Coordinator will be involved with community programs/events that respond to the needs of minority populations who may have multiple health needs and barriers to care.
  • Coordinate and follow-up on referrals to other community organizations that specialize in areas prioritized by individuals.Referrals may include comprehensive and facilitated referrals as well as application assistance and client advocacy.
  • Coordinate with organizations to improve access to services for clients by directly assisting clients in obtaining transportation to appointments, conducting home visits as needed, and participating actively in community, interagency and coalition meetings to foster good working relationships with the community and other service providers.
  • Must be willing to participate in Home visit for potential clients
  • Provide current and appropriate general health and wellness education throughout interaction with clients as well as teaching self-advocacy skills.
  • Identify and document system/service barriers related to connecting clients with health and social services.
  • Complete required productivity reports as needed.
  • Perform individual care coordination work with minimal supervision. May be required to occasionally work irregular hours, attend other job-related meetings, and perform other duties assigned.
  • Participate in regularly scheduled team and inter-organizational meetings
  • Minimum of 1 year experience in a public health and/or social services setting.
  • High School Diploma or GED certificate required
  • Ohio CHW certificate required within 12 months of employment.
  •  Medical Assistant Certification preferred
  • STNA experience preferred
  • A valid driver’s license and current auto insurance required
  • Must remain organized and must prioritize caseloads and communicate effectively to clients, clinicians, case managers and families.
  • Sensitivity and experience in working within different cultures.
  • Ability to communicate orally and in writing in English; fluent in a second language desired.
  • Ability to work independently or with little supervision.
  • Ability to separate personal from professional interactions with clients and maintaining professional/ethical boundaries.

Health Care Access Now provides community-based care coordination services and shared management support services for clients registered with community health centers, private practice providers, health plans, health systems, and social service agencies. The service populations are typically Medicaid and Medicare adults residing in Butler, Clermont and Hamilton counties.
