Requirements for Employment
Registered Nurse in the State of Michigan required
Long term care experience preferred
Strong communication and organizational skills required
Ability to multi-task and provide excellent customer service is required
Knowledge of State and Federal regulatory rules and regulations
Pass a Criminal Background Check, Pre-employment Physical and Drug Screen
Job Summary
Coordinate the admission of new elders to the facility with Social Work to ensure that; elder’s room is set up, medication lists are current and up to date and ordered, coordinate with PCP for any approvals that are needed for orders, and establish elder’s code status. Develop care plan and coordinate elder’s needs to nursing staff and dietary staff. Communicate with elder and their family in addressing any questions or concerns they may have regarding the admissions or discharge process and plans. Complete an assessment, gather vitals, assess skin for any wounds and provide wound care and document accordingly. Coordinate the discharge planning for elders to include; the elder’s home environment, support system, at home services, appropriate education is completed, medications are called in to pharmacy, and referrals are made for at home therapy. Attend Medicare and other appropriate meetings to help address elder’s progress.  Work in collaboration with Social Work, Director of Nursing, Administrator, floor nurses, PCP, and Nursing Secretaries on a regular basis. Maintain an organized and clean work environment.
