Christian Community Health Center Amani House program assists homeless children and youth ages 0-17, and adult women ages 18 and over.
The Early Intervention Specialist conducts evaluations and assessments of children to determine developmental delays and recommends evidence based specific services to help children overcome limitations. The Early Intervention Specialist also provides case management services for all children residing at Amani House Shelter.
Conduct evaluations and ASQ assessments of children residing at Amani House Shelter.
Provide case management and service coordination for each child that supports extant IEP or other plans in collaboration with the parent and school district.
Ensure the maintenance and organization of case management records/files.
Monitor the opening of cases and the process for completing initial assessments.
Ensure timely updates of service planning and follow-up.
Monitor the system of tracking, documenting and following up on referrals
Document all therapies and activities in the progress log in accordance with the service treatment plan.
Monitor each child’s progress toward meeting goals.
Conduct interviews with parent and child at the point of intake.
Coordinate programs recommended for the child that could include physical therapy, group therapy, playgroups, and counseling
Work with children to address behavior problems, communication issues, cognitive delays or physical disabilities
Provides advocacy, support, and linkages/referrals to community resources.
Prepare and submit monthly/quarterly/annual reports on activities of program.
Develop and maintain relationships with community agencies
Ensure program compliance with appropriate regulatory agencies
Attend professional workshops and seminars to enhance skills.
Attend monthly Team meetings and all other relevant program meetings as scheduled.
Maximize client outcomes by collaborating within the department as well as collaborating within the organization.
Participate and monitor the Chicago Children Nutrition Program
Perform mandated reports for DCFS if necessary
Drive company van when needed.
Submit desired time off within the correct time span through agency’s ADP database.
Enter client files/care/activity level information into database (HMIS).
Must be able to work some holidays.
Must be able to work flexible shifts, per the needs of the program/activities. Minimally one Saturday per month is required.
Ensure proper maintenance and cleanliness of child activity room (with the assistance from the custodian/maintenance staff).
Ensure safety precautions are observed throughout all activities and trips (with the assistance of the Residential Support Workers as needed).
Provide support to administrative staff (RSWs, Cook and Case Manager)
Perform other duties as required or assigned.
A Bachelor's Degree in child development, early childhood education, special education, developmental psychology or related discipline with a minimum of 3 years experience is required. An Associate Degree in a discipline related to early childhood development is preferred. The ideal candidate has analytical, negotiating, communication and advocacy skills. Experience serving marginalized populations is preferred. An automobile, valid Driver’s License and vehicle insurance coverage is required. Occasional late or weekend hours may be required.
Must possess advanced computer skills, including word processing (be able to navigate through Microsoft Office, be able to create/ edit forms in Microsoft Excel and Word, and attach documents), data base, and Internet communications.
Knowledge of Homeless Management Information System (HMIS) strongly preferred.
Excellent oral and written communication skills, including grammar and active listening skills.
Ability to write clear, accurate case notes and complete other required case documentation thoroughly and accurately.
Must be able to read, write and speak English fluently.
