POSITION SUMMARY
The Peer Community Reengagement Specialist (PCRS) will be responsible for working with people living with the Human Immuno-Deficiency Virus (HIV) that are recently released from prison and jail, disengaged from HIV primary care, and offering services to ensure that individuals are able to connect to and remain engaged in care. 
The PCRS will work in concert with the corrections case managers to assist clients in reducing barriers to maintain medical care and adherence to medications and HIV treatment.  The PCRS will assist individuals with addressing socioeconomic needs related to employment, job training and housing
 
 
RESPONSIBILITIES
  • Work Collaboratively with all service partners of Safe & Sound Reentry Partnership (SSRP)
  • Receive referrals from CDPH Bridge Program, Discharge planners and staff from corrections clinic
  • Create links with Transitional Case Managers in the jails; Housing Case Managers, Employment Specialist, SRO programs, shelters, Substance Abuse Programs and other medical providers
  • Navigate individuals living w/HIV into medical care and other ancillary services needed
  • Conduct eligibility assessments, develop an individual goal plan
  • Document all client encounters and measure progress towards goals
  • Conduct follow up to clients with missed appointments
  • Maintain appropriate referral sources and contact information for access to community based services
  • Work with clients to link to services for 3 months
  • Participate in all case conferencing, training, and other program activities as requested
MINIMUM QUALIFICATIONS
  • High school diploma or equivalent from an accredited school/program.
  • 2-3 years direct service experience working in the field of HIV service provision.
  • Computer proficiency is required as well as the ability to relate to people of diverse backgrounds, cultures, sexual orientation and gender identity. 
  • Preferred candidates will have experience utilizing the services specific to HIV medical care, housing and employment.
An automobile, a valid Driver’s License and vehicle insurance coverage required.  Excellent engagement and motivational skills are necessary.
