Part-time position
In this position you will assess the strengths and weaknesses of individuals served in their homes and community based sites to assist them with determining appropriate goals. Coaching and supporting individuals by teaching job tasks and daily living skills and encouraging integration in the community, providing orientation and training to assist in the expression of personal preferences, asserting rights, and making choices. Utilizes techniques such as modeling, verbal prompting, visual prompting, etc. Coaching individuals served in self-direction, including responses to dangerous situations, making decisions and choices, and making changes in life activities.
Assisting individuals served with daily living skills and provides assistance with routine household tasks, meal preparation, personal care, self-administration of medication, and other areas of day to day living. 
Implements recommended therapeutic interventions under the direction of a professional or an extension of therapeutic services. Assists with behavioral support strategies including training and assistance in appropriate expressions of emotions or desires, assertiveness, and acquisition of socially appropriate behaviors.
Provides medical and health care services that are integral to meeting the daily needs of the individual such as routine administration of medication. Provides emergency response training including development of best responses to various emergencies.
Provides community access services that explore services available to all people, natural supports available, and developing services and supports that allow full integration.
Completes daily paperwork and turns this information into supervisor so that a comprehensive report can be written on individuals served performance. Keeps accurate records by completing and maintaining required documentation including case notes, activity logs, mileage reports and monthly documentation sheets.
Attends meetings and staffings with referral sources, individuals served and their support teams to review progress of the individual served and build on it. Communicates regularly with ES staff regarding progress of individual served.
Attends meetings and training as required by supervisor or agency. Maintains confidentiality of the service record of individuals served and other sensitive subject matter.
Works varied assigned schedule, exhibits regular and predictable attendance and works early mornings, evenings and weekends, as required to meet workload demands.  
 May be responsible for transporting individuals served.  

Minimum qualifications include:
  • High School Diploma or equivalent, technical or vocational training preferred
  • Knowledge of intervention strategies; emotional, mental and physical disabilities; general principles and methods for working with persons with disabilities; job training methods; vocational and habilitation programming; computer software, applications and programs
  • Ability to exercise patience in working with various needs of  persons with disabilities
  • Ability to work independently demonstrating initiative; exhibit flexibility in work schedule and job tasks
  • Ability to develop and maintain effective working relationships with associates, supervisors, job contacts and general public
  • Ability to maintain accurate and complete work records; accurately post information to written records & maintain confidentiality of sensitive subject matter
  • Skilled in written and verbal communication, time management and organizational skills
