Part-time position; 20-22 hours/week
This position requires extensive travel throughout the day
As the Rural Rehabilitation Coordinator you will be responsible for providing individualized and creative assessments, case management, and referral for rehabilitation, assistive technology, and supportive services to farmers and farm workers with disabilities under the Ohio AgrAbility project.  
Developing assessment materials, forms and evaluation materials for assistance,
assisting education coordinator in education program presentations and coordinating individualized services with partners from The Ohio State University, receiving referrals to the project, scheduling personal interviews, assessing customers, making referrals for services, developing service plan, collecting evaluation data on customer assistance, follow-up with customer as needed. Finding and developing relationships with Assistive Technology vendors and service providers. Maintaining documentation of individual services and reporting on project outcomes.
Continually looking for ways to innovate, actively developing new systems, procedures and techniques, and constantly reviewing and revising existing ones.  Challenging the status quo, and encouraging this in others.  Seek to find how new work training activities can be innovative AND comply with standards and regulations.
Must be able to work independently without need for close and constant supervision.  Able to judge and seek direction or supervision when needed.  
Minimum qualifications include: 
  • Minimum requirement technical degree or certificate in a related field, the ideal candidate will possess a Bachelor’s degree with preference for degree in field of Agriculture or Rehabilitation
  • Rural and agricultural knowledge and experience   
  • Knowledge of agricultural production and equipment required with experience preferred
  •  Knowledge of Assistive Technology solutions and vendors
  •  Knowledge of OOD services and system
  •  Ability to work independently and use good judgment
  •  Flexibility in work schedule 
  •  Ability to effectively communicate both orally and in writing with persons   served, project staff, and a variety of state and private service agencies
  •  Valid driver’s license & insurance & a good driving record
