At Easterseals, discover a rewarding career that makes profound and positive differences in people’s lives every dayAs a CCMEP Case Manager you will be responsible for  providing Comprehensive Case Management and supportive services for youth 16-24 years old, providing assessment of services, documentation and coordination of service plan delivery to meet the performance expectations outline in the CCMEP Scope of Work. Creates Individual Opportunity Plans that address removal of barriers to ensure individual is successful in achieving performance outcomes, managing a caseload of 65+, entering information in two data systems within 24-48 hours of meeting clients. Must be creative in motivating individuals served to engage, retain and succeed in program services. Identifies and creates relationships and opportunities with community agencies to meet the needs of the individuals served. Responsible for documentation of individual’s progress.

Minimum qualifications include:
  • Bachelors in a Human Services or related field 
  • Excellent communication & organizational skills
  • Ability to develop & maintain good public relations
  • Attention to detail, and skilled in multi-tasking
  • Ability to adapt quickly to changing environments
  • Flexibility in work schedule and job tasks
  • Valid driver’s license & automobile insurance & a good driving record
Preferred qualifications include:
  • Knowledge of government assistance
