Job Duties:
  • LPN/RN directs the day-to-day functions of the Certified Nurse Aides (CNA) to include but not limited to: ensure compliance with all current rules, regulations and guidelines that govern long term care facilities; ensure work assignments are performed within acceptable nursing standards; make reports/recommendations concerning the activities of CNA; assign CNA to specific Residents: ensure CNA is aware of the member care plans
  • LPN/RN receives telephone orders from physicians and record on the MAR
  • Assures that appropriate, informative and descriptive documentation/medical records are maintained to include but not limited to: chart all accident/incidents involving members and notify physician; chart member’s admission, transfer and/or discharge; transcribe physician’s orders to resident charts, medications sheets and treatment/care plans; document unauthorized discharges in accordance with established department procedures; report all discrepancies noted concerning established member care to DON to ensure that the nurses' notes reflect that the care plan is being followed
  • LPN/RN coordinates nursing services portion of MDS and Member Care Plans/Discharge Plans with other involved department
  • Ensures that narcotic records are accurate and notify DON of any discrepancies
  • As required administer professional nursing services to include but not limited to: obtain sputum, urine and other lab tests as ordered; take and record TPR and blood pressure; tube feeding; suction; apply and change dressings/bandages, packs, colostomy and drainage bags; Range of motion exercises; care of dying
  • LPN/RN participates in the orientation of new member/family members to the Health Care Center; continue to meet with members/family members as necessary
  • Monitors seriously ill members as necessary; notify the member’s attending physician and next of kin when there is an acute change in the member’s condition
  • LPN/RN assists employees in solving day to day questions and issues as they arise
  • Immediately upon hire, train and/or orient employees in all aspects of their position to include but not limited to: job description/essential functions of the job; minimum acceptable levels of performance; risks and prevention to infectious disease; proper and safe use of work tools, equipment and other procedures; proper hand washing techniques; safety procedures, proper use of personal protective equipment and hazard communications program; universal precautions/blood borne pathogens program
  • Assists other employees in lifting and/or carrying unusually large or heavy objects each shift in a manner that complies with company safety procedures
  • LPN/RN effectively communicates with members, families and Health Pavilion Staff
  • Recognizes changes in a resident’s condition, acts appropriately to these changes, facilitating needed care. Reports all changes to the Director of Nursing
  • Sets up and passes medications, observing the 5 rights of medication administration
  • Greets all new members, completes admission paperwork, including admissions intake form
  • LPN/RN facilitates timely discharges from the Skilled Nursing, providing written and verbal information to receiving facility or home care as needed
  • Attends and participate in continuing education programs designed to keep you abreast of changes as well as to maintain your license on a current status. Attend department, in-service and company meetings as scheduled
  • LPN/RN maintains confidentiality of members’ PHI (Protected Health Information) and medical records information as well.  This position is classified as a level 4 in the ePhi Access Group levels
  • This position is considered to be an essential role and the incumbent may be required to work during emergency situations including natural disasters.
Qualifications:
 
  • Completion of a nursing program at an accredited school of nursing
  • Two years of nursing experience in similar setting is preferred
  • Possession of a current Florida nursing license
  • LPN must be able to work with geriatric members and with memory impaired individuals and groups
  • Ability to work closely with professional and non-professional staff members
  • Demonstrates effective supervisory, communication (oral and written) and interpersonal skills
  • LPN demonstrates skills and abilities in clinical and administrative areas including ability to work with limited supervision and to make decisions based on established policies and procedures
  • This individual will need to be empathetic, energetic and have an affinity for working with a diverse senior population
  • All applicants must successfully pass a drug screen and Level II background check
